By acting as a prompt for clinicians to discuss meaningful issues that affect individual patients, the Toolkit builds better patient-clinician relationships, changing a person’s experience within the healthcare system. It also encourages important insights into better care provision for people with other long-term health conditions. The Toolkit represents the individual and their changing experience, personalising the interaction a patient has with their clinician and encouraging clinicians to be empathetic and to view their cognitively-impaired patients holistically.
